Accessing Services

The Integrated Adult Services Directorate is committed to helping people to live as independent a life as possible in the Community.  To do this we ensure that people receive the support services they really need.

Assessing your needs

A Social Worker or Nurse will assess what you need  to support you to continue living in your own home or we may look at other ways to meet your needs.  To do this the Social Worker or Nurse will complete a Unified Assessment. 

To gather information on what your needs are we will do the following:

  • We will ask you about your life and how you want support to help you cope;
  • We ask your husband / wife or any other partner or carer about what they think you need to help you cope;
  • From any health professionals or support workers you may already have support; with your permission we will find out about the way you like to spend your time now and, if you had help, what you would like to do in the future. 

The things we look at are:

  • Your health
  • Your physical needs

            e.g.    climbing the stairs
                       mobility in and outside your home
                       washing and bathing
                       dressing
                       eating etc.
          

  • Shopping
  • Keeping warm
  • Your senses
             

            e.g.    eyesight
                       hearing, etc

  • Your state of mental health
  • Your relationships with others, including support groups like the Church
  • Your safety
  • Your home
  • Your finances
  • How you spend your day.

How we plan your support

Using this information we will be able to agree your support needs.  The Social Worker or Nurse will prepare a plan of how we, other carers and your family will all work together to help support you.  This is called a Care Plan.

The services will not be arranged without your agreement.  The Individual Care Plan is yours and the decisions are yours, made in partnership with us and other professionals if required.  Together we will review your Care Plan at least once a year and it will be changed as your needs change.
